We are located at 23745 225th Ave SE #215 in Maple Valley (on the second floor above La Fogata Mexican). Dr. Amy will be waiting for you and will come out to greet you as soon as she has finished with the previous patient.
Prior to your first appointment, please print out New Patient Forms, complete them, and bring them with you. You also may find it helpful to write out a history of your current injury or pain — that way you’ll be sure not to forget anything. This written history isn’t required, but may help us to fully cover any issues that you have been struggling with for a long period of time.
Appointments & Cancellations
Please arrive on time for your appointments to ensure we are able to complete your full treatment at each session. If you are unable to keep your appointment, please let us know as soon as possible. We require 24 hours notice except in cases of emergencies.
Payment is due at time of service via cash, check, HSA account, or credit card.
Upon request, you can receive receipts with treatment codes to bill your insurance directly. Insurance plans and coverages vary — we recommend that you contact your insurance company to verify your benefits prior to treatment if you plan to seek reimbursement. To make it easier, we’ve provided a list of Questions to Ask Your Insurance to help you understand your physical therapy benefits.
If your insurance requires pre-authorization, please let us know via e-mail as soon as possible at firstname.lastname@example.org. If your insurance requires a prescription, you will want to attach this to each claim you submit to your insurance company.
What to Wear
Please dress in comfortable clothing that you can move easily in — clothing that allows for visibility and direct treatment of your area of injury or pain. Most treatment sessions will end with a specific exercise prescription for you to continue until your next treatment session.
